Make the task specific

A follow-up needs a reason and a date, not just a name on a list. Use the clinician's plan to record what needs to happen. The clinic must decide who reviews the worklist and who handles clinical questions.

Record the outcome of each attempt

Separate reminders from follow-up responsibility

Email reminders depend on the recorded agreement and a usable address. An invitation creates portal access; it is not a reminder preference. A sent message does not prove that the patient read it, returned to the clinic or completed the doctor's plan.

A reminder is a communication event. The clinic should record the relevant outcome and decide whether the task is complete; a message alone is not evidence of completed care.
